Challenge Summary

Welcome to HTC - Enrollments Form Storyboard Design Contest. In this contest we need your help to design screens for our application. The existing application is in a wizard format that required some screens. We would like to combine them into one or just a few screens so the application will looks more user friendly, intuitive, clean, professional and meet the current look and feel of public Humana website.

All required screen plus any feedback from milestone phase.


Project Overview

Humana is a managed health care company that markets and administers health insurence. In this contest Humana and TopCoder (HTC) have teamed up to convert Humana enrollment form application to use another technology. The first step is update look and feel of this application and making the flow more user friendly. This application will be used by their agents for enrolling their customers. We provide you existing screen of application, we need your help to come up with your idea to make this application has intuitive flow, looks professional and modern and also follow look and feel of Humana website.


Contest Details

We provide you existing screens of application. All screen of application is attached in contest document. Following are flow details of the screens:

- Print enrollment change form (1). This is the 1st step of application.

- Print enrollment change form (2). This is the screen that shows 2nd step of application. Modify existing group information or plans options (2) is screen that shows options when Modify existing group information or plans options is selected. Modify an existing employee's coverage options (2) is screen that shows options when Modify an existing employee's coverage options is selected.

- New group enrollment (3) is the screen that shows 3rd step of application when New group enrollment in 2nd step is selected.

- Add a new employee (3) is the screen that shows 3rd step of application when Add a new employee in 2nd step is selected.

- Add a new line coverage (3) is the screen that shows 3rd step of application when Add a new line coverage in 2nd step is selected.

- Modify existing group information or plans (3) is the screen that shows 3rd step of application when Modify existing group information or plans in 2nd step is selected.

- Modify an existing employee's coverage (3) is the screen that shows 3rd step of application when Modify an existing employee's coverage in 2nd step is selected.

- All screens in 3rd step that have postfix "UNCHECKED" is screen that shows state where the checkbox in that screen is not checked.

Based on provided contest document and contest details, we need your help to design new storyboard for this application so the application has intuitive flow, more user friendly as well as meet look and feel of Humana public website.

 

Branding Guidelines

- Follow provided example look and feel screen. It is attached in contest document.

- Content width size is 1024px.

- Font and colors is up to designer, but should match provided example look and feel screen.

 

Target Audience

- Humana's agents.

 

Judging Criteria

- Better application flow.

- Completeness, cleanliness, creativity and graphic appeals.

- How well your submission meets the goal contest and follow look and feel of public Humana's website.

 

Submission Deliverables

Following are all items you need to provide in your submission:

Preview Image

Create your preview image as one (1) 1024x1024 JPG or PNG file in RGB color mode at 72dpi and place a screenshot of your submission within it.

Submission File

Please upload PNG/JPEG images in a zip file with all requested contest requirements stated above.

Source Files

All original source files of the submitted design. Files should be created in Adobe Photoshop or Illustrator (or equivalent) and saved as layered PSD/AI files.


Final Fixes

As part of the final fixes phase you may be asked to modify your graphics, images (sizes or colors) or modify overall colors.

Please read the challenge specification carefully and watch the forums for any questions or feedback concerning this challenge. It is important that you monitor any updates provided by the client or Studio Admins in the forums. Please post any questions you might have for the client in the forums.

Stock Photography

Stock photography is not allowed in this challenge. All submitted elements must be designed solely by you. See this page for more details.

How To Submit

  • Upload your submission in three parts (Learn more here). Your design should be finalized and should contain only a single design concept (do not include multiple designs in a single submission).
  • If your submission wins, your source files must be correct and “Final Fixes” (if applicable) must be completed before payment can be released.
  • You may submit as many times as you'd like during the submission phase, but only the number of files listed above in the Submission Limit that you rank the highest will be considered. You can change the order of your submissions at any time during the submission phase. If you make revisions to your design, please delete submissions you are replacing.

Winner Selection

Submissions are viewable to the client as they are entered into the challenge. Winners are selected by the client and are chosen solely at the client's discretion.

Submission format

Your Design Files:

  1. Look for instructions in this challenge regarding what files to provide.
  2. Place your submission files into a "Submission.zip" file.
  3. Place all of your source files into a "Source.zip" file.
  4. Declare your fonts, stock photos, and icons in a "Declaration.txt" file.
  5. Create a JPG preview file.
  6. Place the 4 files you just created into a single zip file. This will be what you upload.

Fonts, Stock Photos, and Icons:

All fonts, stock photos, and icons within your design must be declared when you submit. DO NOT include any 3rd party files in your submission or source files. Read about the policy.

All submissions are screened for eligibility before the challenge holder picks winners. Don't let your hard work go to waste. Learn more about how to  pass screening.
