DocuSign: Update SOAP SDK Samples 1: PHP

Let's get started:

• Get a developer account from DocuSign DevCenter (www.docusign.com/devcenter)

���Requirements:

• Fork the current git repo (https://github.com/docusign/DocuSign-eSignature-SDK/tree/master/PHP),
  • update proxies,
  • make sure that the style sheets all work
  • make sure that that you can successfully deploy it using the instructions included.
• Fix any bugs that you find.
