Challenge Summary

Welcome to "DST - Mobile Diagnostic Survey Tool Design Challenge". This challenge purpose is to create look and feel for our upcoming survey tool which will be used in both iPad and iPhone devices. We are looking for your design expertise to help us create storyboard for this application. Jump in NOW!!

Round 2

All requirements as stated in challenge details with client feedback applied.


This challenge purpose is to create look and feel for our upcoming survey tool application which will be used in both iPad and iPhone devices. We are currently don't have any specific design guidelines, so you are free to create and apply your design style in this challenge. Please read more details below : 

General Requirements :
- All screens should include a header and footer.  
- Header should include a logo placeholder and title "Diagnostic Survey Tool".  
- Footer should just contain copyright notice and a small link to "Our Site".

Screen Requirements :
1. Intro screen (iPad)
- wf references is 01.jpg. 
- This screen will be showed up by default when user open the application. 
- We need this screen to be attractive.  
- Please include a way (maybe a button, or else) to start choosing surveys.

2. Survey Type (iPad)
- wf references is 02.jpg. 
- the first step in selecting a survey.
- there will be many surveys that application user can choose.
- this screen needs to be attractive.  

3. Survey Element (iPad and iPhone)
- wf references is 03.jpg. 
- the second step in selecting a survey (shows the previously selected type)
- show us which survey that has been done and which one still available for vote.
- there will be multiple survey type here, please create design that able to accommodate it.

4 and 5. Survey questions (iPad and iPhone)
- wf references is 04.jpg and 05.jpg. 
- These pages show the selected Type and Element.  
- There is a list of Themes (groupings of questions) displayed and the user can choose to skip to any theme in the survey (eg. Customer Outreach).  
- Each Theme will have a number of questions (04 shows that Priority and SWOT Theme has 5 questions, 05 shows that the Innovation and Agility Theme has 11 questions).  
- One question is shown at a time and you can swipe to continue to the next question in the list.  
- After the last question there will be a submit survey button (shown on 05 - if you think this should be a separate page instead, you can create your design that way).  
- There are 4 types of questions that needs to be create: 
* Priority (where the user drags/drops multiple choices to put them in a certain order on the screen) shown in 04, 
* Multiple Choice (choose 1 from X choices), 
* Text Field, 
* and Yes/No shown in 05.  
- Each question will show some question text and then the user input (Yes/No buttons, text field, etc.)  
- Also there will be a small "Notes" icon, when pushed it will show/hide an optional text field for each question.  This way the user can enter notes about their answer if they want.

6. Submit Survey (iPad)
- wf references is 06.jpg. 
- When the user submits the survey, they are asked to enter their email address.  
- Once they have done that, they have the option to click on "Report Results" to show the results.  
- Prior to entering a valid email, they cannot click on that main button.  
- There is also an option to go back to the survey and change your answers.

7. Report Results (iPad and iPhone)
- wf references is 07.jpg. 
- The screen shown for this page is not too accurate so be sure to read and follow this description carefully instead.
- The report will consist of 3 sections which can be scrolled through.  
- The first section will be called Business Considerations and will contain about 2 paragraphs of text explaining what recommendations the app makes.  
- The next section will show a Considerations List, which is a list similar to what is shown in 07 (it should not say category and should not display a score, but should be a list of items in priority order).  
- The final section should be called Survey Details and should show all of the questions and answers and notes that the user entered in their survey.

Branding Guidelines
- Font    : Open to designer
- Color    : Open to designer
- Style    : Open to designer
- Create it for two different devices (portrait) :
* iPhone    :  640 x 960 resolution 326 ppi
* iPad        : 1536 x 2048 resolution at 264 pixels per inch (ppi)

IMPORTANT:
- Keep things consistent. This means all graphic styles should work together.
- All of the graphics should have a similar feel and general aesthetic appearance

- How well you plan the user experience and create your ideas visually
- Cleanliness of your graphics and design
- Your design should possible to build and make sense as a mobile application

Submission & Source Files
Preview Image
Please create your preview image as one (1) 1024x1024px JPG or PNG file in RGB color mode at 72dpi and place a screenshot of your submission within it.

Submission File
Submit JPG/PNG for your submission files.

Source Files
All original source files of the submitted design. Files should be created in Adobe Photoshop and saved as layered PSD file.

Final Fixes
As part of the final fixes phase you may be asked to modify your graphics (sizes or colors) or modify overall colors. We may ask you to update your design or graphics based on checkpoint feedback.

Please read the challenge specification carefully and watch the forums for any questions or feedback concerning this challenge. It is important that you monitor any updates provided by the client or Studio Admins in the forums. Please post any questions you might have for the client in the forums.

Stock Photography

Stock photography is not allowed in this challenge. All submitted elements must be designed solely by you. See this page for more details.

How To Submit

  • Upload your submission in three parts (Learn more here). Your design should be finalized and should contain only a single design concept (do not include multiple designs in a single submission).
  • If your submission wins, your source files must be correct and “Final Fixes” (if applicable) must be completed before payment can be released.
  • You may submit as many times as you'd like during the submission phase, but only the number of files listed above in the Submission Limit that you rank the highest will be considered. You can change the order of your submissions at any time during the submission phase. If you make revisions to your design, please delete submissions you are replacing.

Winner Selection

Submissions are viewable to the client as they are entered into the challenge. Winners are selected by the client and are chosen solely at the client's discretion.

Submission format

Your Design Files:

  1. Look for instructions in this challenge regarding what files to provide.
  2. Place your submission files into a "Submission.zip" file.
  3. Place all of your source files into a "Source.zip" file.
  4. Declare your fonts, stock photos, and icons in a "Declaration.txt" file.
  5. Create a JPG preview file.
  6. Place the 4 files you just created into a single zip file. This will be what you upload.
