Checkpoint requirements are only TWO screens, looking forward to your designs!

Round 1

Please submit your designs for at least these two screens for checkpoint review:
- Procedures

Challenge Details
This is a UI/UX redesign for an existing surgical procedures app. Their clinical sales reps have a need to capture procedure data once a surgery is performed using their devices. They have an existing mobile App that captures that data. However, we are looking to help them improve the overall UI/UX to increase adoption of the mobile app.
In this challenge, we are looking for a visually appealing, and easy to navigate user experience that supports all the given requirements.

Design Concept Goals:
- Do not simply re-skin the existing design! Think about the information and how it should flow, and how to capture surgical data in a user-friendly way.
- Suggest screen flows, navigation and screen effects.
- How quickly can you find information?
- Think simple but effective solutions

Design Requirements:
- Create it for iPhone 5 Portrait orientation (640 x 1136 resolution 326 ppi)
- Please adhere to iOS 7 design guidelines: https://developer.apple.com/library/iOS/documentation/userexperience/conceptual/mobilehig/
- Please leave a placeholder for the logo

Screen Requirements:
- See existing screens: Current-App.pdf in Downloads, for the content and how the app currently works.
- We are looking for sleek, minimal, clean design.
- Keep in mind that we are open to changing the flows adding/removing screens as you (the designer) think is needed as long as the overall requirements are met. Just use the existing screenshots as a guide to the content, but completely rethink the screens and UX for the app.

1. Login Screen
See page 1 of Current-App.pdf
- Username and Password fields need to be input
- Forgot Password text

2. Procedures (inlucing filtering/sorting procedures)
See page 1 and 2 of Current-app.pdf
- Should this be the first screen the user sees after login
- Procedures are grouped by hospital, or by case date. This needs to be shown somehow. The data can also be sorted according to either hospital or case date.
- This is a list of procedures, with these fields shown:
—Hospital ID
—Case Date
—Procedure ID
- Show filter options
—Onsite
—Completed
—Cancelled
—All
- Show Filter-On state (and how to reset filters).
Currently the app only shows onsite procedures by default.

3. Procedure Detail
See page 1 and 2 of Current-app.pdf
- Once a case is selected from procedures list, the procedure details will be shown
- There should be an "Edit" option
- The following fields will be displayed:
—Case Date
—Status
—Hospital
—System
—Console Surgeon
—Category
—Procedure Name
—Procedure Notes
—Fluorescence Imaging

4. Create New Procedure
See page 3 of Current-app.pdf
- Show mandatory fields with an asterisk, or any effective way to ensure completeness
- The fields will be the same as those listed in Procedure Detail (above)

Fonts and Colors:
Typography:
- Use Helvetica fonts only (you may use Arial if you don't have Helvetica)
- Large, easy to read text
- No bold or italic text (use font color and size to define hierarchy)

Colors:
- Light background color preferred
- No text should be darker than #333333 or lighter than #999999
- Client colors are:
— Light blue : #e2eff8
— Yellow : #fbb901
— Grey : #89969e
— White : #FFFFFF
- We would love to see some nice large / clean icons on this.
