Welcome to "Chiron - Responsive Health Report Design concept challenge". In this challenge we need your help to create design concepts for our health report application which will be use to inform patients about their medical information. Only 4 screens need to be created in this challenge, so Join Now and Grab the main prizes!!

Concept Description
Our client wants to build a mobile application that showing health information in a usable and easy to read format to patients so they will understand their medical information. There are 4 types of medical information that we want to create in this challenge, and we need your creative thoughts on how to create and presented these medical information to patient.

General Requirements :
- We need navigation between these views that is easy to understand and works well for the screens and their content
- These screens are to be created for patient view, consider patients with vision problem or medical issues.
- Please include a loading screen and login screen as a part of your design concept
- They should be created as read-only without editing functionality.

- However, please design with extensibillity in mind!  The client may want to expand the functionality in the future, so make your designs in a way that screens, functionality, interactions and navigations have room to grow as the application may grow!

Screen Requirements :
There are 4 screens needs to be create in this challenge, they are :

01. Summary Page - Complex Case Management (CCM) Screen
- Content should be taken from Summary Page Complex Case Management.pdf
- Provide a general overview of all pertinent information related to the patient’s CCM status
- Displays the recommended program, clinical profile, social & family profile, assigned Interdisciplinary Team (IDT), education history and outstanding followup tasks

02. Care Plan Screen
- Content should be taken from Care Plan.pdf
- Care Plan is a tool to document and address the individual needs of a patient based on their health conditions.
- A comprehensive care plan identifies goals for the patient, according to their needs and priorities, and barriers in achieving those goals.
- This page helps to monitor the progress of a patient and is continuously used to reassess the needs of the patients in terms of clinical, behavioral, social and cultural necessities.
- Care Plan gives the ability to document chronic conditions, pain assessments, assess mental health, activities of daily living, life planning activities, caregiver resources, and community resources.
- Follow up tasks can be assigned to various IDT members who are involved in providing care to the patient.
- Links to various assessments like PHQ2, PHQ9, Home Safety and Mental Health are provided, which can further be printed out as a survey and/or narrative, and provided to the patient or the PCP.

03. Summary page - Disease Management (DM) Screen
- Content should be taken from Summary Page Disease Management.pdf
- Summarize all pertinent information related to disease management for a specific patient.
- The summary identifies the recommended disease programs and the patient's enrollment status.
- It also identifies the criteria to be managed/followed up for each disease management program.
- For each criteria, the goal and the latest values are shown.
- The case manager, after reviewing the information, can document the actions taken and set up future follow up task.
- The case manager can also complete any open follow up tasks.

04.Medication Reconciliation Page Screen
- Content should be taken from Medication Reconciliation.pdf
- Used to document all of the medications that the patient is currently taking. It allows the user to review the patient's history of prescribed medications and add specific medications to the current list. It also allows the user to search and add for a specific medication from a comprehensive data warehouse
- Contraindications are displayed to reduce potential/actual adverse drug interactions which may cause harm to the patients
- Follow up action items can be assigned to IDT
- Attestation is required by an IDT that is a physician, pharmacist, physician assistant, nurse practitioner or registered nurse
- After a recent hospital discharge, a post discharge medication reconciliation letter and form can be generated in PDF for the provider to review and make any appropriate changes

Design Considerations
- Design it for mobile but keep Responsive Design in mind 
- Make sure your design is clean, good looking and very usable.
- Consider create a design that is easy to navigate, Information needs to be arrange in some appropriate ways so it is easily to figure out sets of information.
- Give importance to the overall layout and think on how a user would interact with the content on the page.
- For mobile, please use latest iPhone size (1136-by-640 resolution).

Branding Guidelines
- Fonts : Open to designer, make sure your font choices is very easy to be read for our audience.
- Colors : Choose a palette that is both good-looking and enhances the usability of the application
- Styles :Open to designer.

Target Audience
- Patients, strong consideration towards ACCESSIBILITY, adjusting fonts/controls for people with vision problems or medical issues.  

Judging Criteria
- How well you plan the user experience and capture your ideas visually
- Cleanliness of your graphics and design
- Design and User Experience.
 

Submission and Source Files

Preview Image
Please create your preview image as one (1) 1024x1024px JPG or PNG file in RGB color mode at 72dpi and place a screenshot of your submission within it.

Submission File
Submit JPG/PNG for your submission files.

Source Files
All original source files of the submitted design. Files should be created in Adobe Photoshop and saved as layered PSD file, or Adobe Illustrator as a layered AI file.

Final Fixes
As part of the final fixes phase you may be asked to modify your graphics (sizes or colors) or modify overall colors. We may ask you to update your design or graphics based on checkpoint feedback.

Please read the challenge specification carefully and watch the forums for any questions or feedback concerning this challenge. It is important that you monitor any updates provided by the client or Studio Admins in the forums. Please post any questions you might have for the client in the forums.

  • If your submission wins, your source files must be correct and “Final Fixes” (if applicable) must be completed before payment can be released.
  • You may submit as many times as you'd like during the submission phase, but only the number of files listed above in the Submission Limit that you rank the highest will be considered. You can change the order of your submissions at any time during the submission phase. If you make revisions to your design, please delete submissions you are replacing.

Winner Selection

Submissions are viewable to the client as they are entered into the challenge. Winners are selected by the client and are chosen solely at the client's discretion.

Your Design Files:

  1. Look for instructions in this challenge regarding what files to provide.
  2. Place your submission files into a "Submission.zip" file.
  3. Place all of your source files into a "Source.zip" file.
  4. Declare your fonts, stock photos, and icons in a "Declaration.txt" file.
  5. Create a JPG preview file.
  6. Place the 4 files you just created into a single zip file. This will be what you upload.
