Design Problem – Health System Collaboration across Patients, Physicians, and Hospitals
We are looking for design concepts for a modern enterprise collaboration tool that includes:
iPhone portal for patients
- iPad portal for physicians
- Desktop portal for health system executives.

The collaboration tool you are designing will empower all three user groups to share information in real time, use analytics to analyze risks (for individual patients, across a physician’s group of patients, and across the entire health system), and uses these analytics to alert all three groups to situations requiring action. By supplying useful information about health behavior of patients and near real-time notifications from patients and physicians, the app helps the entire health system provide better care for its patients, minimize risk across the Collaborative, and encourage patients to continue getting health care services from the health system.

A “Health Collaborative” is a collection of regional health systems that have partnered together to provide higher quality health care for their community and minimize financial risk.

Scenario:
A Collaborative of regional health systems is considering ways to assume financial risk, improve the health status of the communities they serve, while driving long term market share and financial sustainability.
- This organization needs tools that executives can use to understand dynamic physician networks, performance, overall utilization, quality and financial implications.
- We want to empower their physicians with actionable information at the point of care to inform clinical decisions.
- Patients need tools to manage their health and make it easy / seamless to access the healthcare system. Health systems that provide these tools for patients can improve their overall brand and patient retention.

Branding Requirements
Color Palette:
- See attachment: Palette.jpg

- Please use the following primary colors when possible.
- Choose your own fonts but please remember to not go too crazy since this is for a hospital and possibly older patients!

Target Audience Style and Inspiration:
- Patients that need help managing their diseases
- Physicians who are very busy and want quick access to information they can use to provide better care for patients
- Health System Executives that need real-time information and analytics to help them manage their healthcare facilities.

Challenge Requirements and Screens
Patient View
- Mobile iOS (iPhone) - Portrait View
- Design screens for iPhone 6
- 750x1334 (You do not need to create Retina - whatever "scales" for iPhone 6)
- Native iOS8 application. This must be designed for iOS8!

Patient Screen 01: Mobile
Patient logs into her iPhone app to get her own personalized content, notifications, and health tracking.  Many of these patients have chronic conditions that must learn how to manage many of the basic care/treatments for their disease. This app will serve as a tool for patients to get information about their disease, track their daily habits, get coaching advice (either from their physician or through artificial intelligence/data analysis engines), and ultimately empower them to take control of their disease!

The patient home screen should be an interactive and user-friendly dashboard to explore/ navigate 3 main areas:
1. Lifestyle
2. Self-Management
3. Access Care

The patient dashboard should allow her to quickly see important information for each section:
- Notifications
- Urgent alerts
- Gamification elements for targets achieved (such as weight loss or exercise)
- Medication reminders, etc.

Patient Screen 02: Mobile
- The priority view for the Patient mobile app is the ‘Self-Management’ navigation item (#2 above).
- From this screen, the patient should be able to monitor her blood sugar (past measurements and connect a device to her phone to take a real-time measurement), monitor her blood pressure, and review currently prescribed medications, and track other health related activities.
- We need your ideas for how this can be very visual and user-friendly, while also being informative for the patient.

Patient Additional Details and Thoughts
- You can find additional details for the types of data a patient could expect to see in all three sections of the Patient Mobile App in the attached Patient View Spec Sheet.

- In addition to the features listed above, we’re really excited to see how you think patients and physicians should collaborate in the future!  Should they be able to FaceTime directly from the app?  Should the patient be able to request a prescription refill with the press of one button?  Please give us your creative thoughts when designing these screens!

Physician View

- Mobile iOS (iPad) - Landscape
- Design screens for iPad 2 landscape display
- 2048x1536 (You do not need to create Retina - whatever "scales" for iPad 3/4)
- Native iOS8 application. This must be designed for iOS8!

Physician Screen 03: Tablet
Physician logs into his iPad app to get his own personalized dashboard of his patients and other relevant information.

- The physician home screen should be a visual and user-friendly dashboard that presents the physician with actionable information.
- The objective is to give the physician real-time information that enables him to provide better care for his patients and make his job easier.
- We think there should be two major sections to the Physician Dashboard:

1. Patient Action Opportunities
- What patients is he seeing that day?  Does he have any alerts or actions needed before seeing them?
- Does he have any urgent / emergency patient needs?
- Does he have any outstanding prescription refill requests?

2. Quality and Cost Performance
- Are the physician’s quality metrics increasing? How do his costs per patient compare to the average at his Hospital System? How do they compare to the average in the larger Collaborative?  How do they compare to the industry average across the country?
- This section is a quick snapshot to help the physician easily see what he can do better, and what he’s doing well right now.

- The physician should be able to access features such as advanced analytics and artificial intelligence to predict patient behavior and needs so that he can focus more of his time on preventative care rather than responding to emergency care.
- Please see the attached Physician View Spec Sheet for more specific details on the data / information the physician could see on his dashboard.

Physician Screen 04: Tablet
- The priority view (Screen 04) for the Physician mobile app is a deeper look into the ‘Quality and Cost Performance’ screen described above.
- This screen should help the physician understand how he is impacting quality of care, cost of care, patient retention, etc.  Ultimately, it should tell him what he needs to continue doing and what he needs to do better.

Physician Additional Details and Thoughts
- Important: we need you to think about how your design can help a physician make decisions. For example, how can the information on this screen help him direct patients to the lowest cost, highest quality outcome?
- You can find additional details for the types of data, reports, and analysis a physician could expect to see in on this screen in the attached Physician View Spec Sheet.
- In addition to the features listed above, we’re really excited to see how you think patients and physicians should collaborate in the future!  Should they be able to FaceTime directly from the app?  Should the physician be able to intervene and communicate directly with his patient if he sees that she hasn’t taken her medicine today?  What sort of data analytics do you think a physician would want to see to provide better care for his patients?  Please give us your creative thoughts when designing these screens!

Health Executive View

- Desktop – Landscape
- Design screens for a desktop display (1280x1064 resolution; 4:3 ratio)

Executive Screen 05: Desktop
A Health Executive wants to see how her health system, as well as the larger Collaborative, is performing and whether physicians are providing high-quality care to patients.  The Health Executive home screen should be a high-level dashboard that provides a quick snapshot of the overall performance of the health system.

We envision two major sections for the Executive Dashboard:
1. Easy / quick access to the many reports and analysis available from data gathered throughout the their Health System (including patient and physician data) and the larger regional Collaborative.
- Please see the Health Executive View Spec Sheet for specific reports and analysis available to the Health Executive.

2. Two or three interactive visual representations that will alert the Health Executive to major issues or problem areas.
These may include:
- A hot spot chart showing problem areas across “Utilization”, “Quality”, “Operating Costs”, “Revenue”, and “Patient Retention”
- Visual representation showing any geographical or patient type patterns in quality of care
- See the Health Executive View Spec Sheet for visuals that you can use for this section

There are two priority views for the Health Executive:
Executive Screen 06: Desktop
- Priority view (Screen 06): High Value Network Performance and Optimization
- In this screen, the Health Executive would want to see information, visuals, data, etc. that show how Primary Care Physicians (“PCP”) and Specialists (“SCP”) are distributed throughout the system.
- (See visuals in the Health Executive View Spec Sheet)
- The Health Executive might be able to layer on additional information to this visual such as adding performance information for physicians (low or high performing PCPs, low or high performing SCPs).  Another big area of interest for both physicians and Health Executives is patient in network retention (making sure patients stay at her health system instead of going to a different one to get healthcare).
- Be creative when thinking about this screen.  If you are managing multiple physicians and want to understand which physicians are providing the best care, what data or information would you want to see?
- Would you want to be able to filter and/or search for specific hospitals within the network?  Filter and/or search for specific physicians?  Filter by PCP/SCP? Filter by patient type? Filter by geography?  There are many options to choose from here.

Executive Screen 07: Desktop
- Priority view (Screen 07):  Financial Performance and Health System Management
- Even though providing high-quality patient care is the first priority of the Health System, the Health Executive is also interested in monitoring financial performance of the system, including risk sharing between members of the Collaborative, resource utilization, revenue, and understanding how performance and revenue are distributed across members of the Collaborative. Managing financial performance and risk is relatively new for these executives, so it will be useful for them to have intuitive, simple, management support!
- When designing this screen, think about where the hospital system can use predictive analytics to help the Health Executive manage operations.  Can she view predicted monthly and annual cost, revenue, physician utilization?
- One of the primary concerns of the Health Executive is understanding her health system’s performance compared to other health systems.  Are her costs higher or lower? Do her physicians rank higher or lower (called a “PMPM Rank”)? What is her Average Cost of Care compared to her Target Cost of Care?
- Can the health system use artificial intelligence (such as IBM Watson) to identify and recommend opportunities to decrease costs, reallocate physicians to underserved geographies, increase quality of care, etc.?
- Again, please be creative with this screen!  Part of this design exercise is to get the perspective of creative people outside the normal healthcare environment.
- Please see the attached Health Executive View Spec Sheet for additional details and visuals.
