Concept Description:
We would like your help in designing an app to allow users to put all their health information in one place. They will be able to upload their medical records, input medical information manually, as well as provide username and password information for medical records as well as fitness tracking devices, nutrition tracking apps, genetic testing results, and health insurance.  These users will then receive personalized recommendations to improve health, prevent disease, and save money on medical costs. Users will also be able to create health goals as well.

We are looking for the [topcoder] design Community to help us with planning our new "user experience" (UX).

Design Guidelines:
- Use client logos for colors and branding.

Supporting Documents:
- Mobile Wireframes (Home TH mobile app-5.pdf.zip), please note this wireframe is to show the functionality of the app, the color scheme and layout do not need to be the same.
- Desktop Wireframes (web_site_hard_copy.pdf.zip), those are only provided for reference.
- Health Quiz (health_grade_quiz.xlsx.zip)
- Client Logos (Logos.zip).

Design Goals:
- How should this application be designed?
- How quickly the user can find what he wants? What should the priority features be?
- How quickly can the user understand and interact with the app?
- The application should be easy to use.
- Think simple but effective solutions! Have fun creating this - let us know how you think it should work!

Screen Sizes:
- Mobile Resolution: Design for iPhone 6 Retina Display 750px x 1334px.
Make sure you create graphic in 'shape' format, so when we resize graphics will still look sharp! It will be great if you can provide the landscape version.

Main Features:
1. Post milestones from goals on Facebook (they can sign up for the app with Facebook profile to have that connection).
2. All medical and health information in one location.
3. Personalized recommendations for health improvement and medical cost savings.
