Welcome to Patient Check-in Tablet Application Design Challenge!

In this challenge, we are looking for topcoder designers to come up with innovative designs for a check-in that will allow hospitals to help with the registration of patients, help patients in initiating payments after treatments and also allows them to provide feedback about the check-in process and treatment given to them.

About this application:
The application is focussed on tablets and is used for patient registration at various check-in facilities within the hospitals. It is also used to sign the forms and collect the payments.

User Persona:
Peter is sick and walks to a hospital to get himself checked. Every hospital has different procedures to register a new patient, even this hospital does that but eases the entire process by asking them to enter the details within an application, this tablet app is designed such that it provides a efficient way to enter required details.
- Peter is happy with the method this hospital has employed.
- Application walks through and he is asked questions about himself and few more simple details before he could take the treatment.
- After his treatment, he is presented with the invoice and payment options in the application (pay on the tablet with a credit or debit card (or) see a registrar and pay with cash or check (or) see a registrar and discuss payment options) to choose from and he is able to initiate the payment within the app.
- He decides to pay by credit card, and chooses that option...he scans the card in the app and enters other required details to initiate the payment.
- Once payment is successfully done, he is shown a feedback form that ask his experience about the treatment and check-in process, as he is very with the process - he provides positive feedback and signs off.

Design Considerations:
- Branding is open to designers, just make sure to keep it professional.
- Make sure you create graphic in 'shape' format, so when we resize graphics will still look sharp!
- Feel free to suggest thoughts that can bring good User Experience for this application.
- Colors need to be such a way that even patients with color blindness, vision issues.
- User might not be in mood for typing too much details as they are sick and weak or some might be kids, some in emergencies etc...so need to make sure the overall check-in process is simplified.
- Have your designs of size 2048 x 1536 px (iPad Landscape)

Required Screens:
Below are the list of screens required for this challenge

00 General:
At any point during the check-in process, the patient can:
- Return to the previous screen using back button
- Cancel the check-in using Cancel button
- Need a way for the request help at any point of during the check-in process, think on how this can be done?
- Provide a way to change the language

Check-in Process:
Steps listed below need to be in the below order, looking for your help to:
- think of some ways so user doesn’t feel that it’s too much information to fill in the check-in process.

01 Choose User Type:
- We need to way to know to allow the user to select whether they are a first time user or had visited the hospital before and is on a follow up visit.

02 Demographics:
- Below details are captured: First Name, Middle Initial, Last Name, Date of Birth, Gender, Last 4 of SSN

03 FPOC Questions:
- This is first point of contact questionnaire.
- Below are the list of questions that need to be answered by the first time patients

1. Have you or a close contact traveled outside the US in the last 3 weeks?
2. Please select the countries you’ve visited
3. Please list if you have visited any other countries
4. Have you ever had TB or a positive TB skin test?
5. Had recent close contact with a person who has TB or influenza like illness?
6. Fever greater than 100.4 F or 38.0 C?
7. Cough not related to allergy or COPD?
8. Persistent Cough GREATER than 3 weeks?
9. Cough with Blood Produced?
10. Sore Throat?
11. Night Sweats?
12. Unexplained Weight Loss?
13. Fatigue?
14. Body Aches?
15. Rash?
16. Nasal congestion unrelated to allergies/sinus infections?

04 COA (Condition of Admission) /COS Forms Screen:
- This is terms and conditions (sample) which can be accepted and digitally signed by patient.

05 Payment Options Screen:
After the treatment, User will be shown the invoice and they will have 3 options to choose from for making a payment:
- Pay on the tablet with a credit or debit card.- See a registrar and pay with cash or check.
- See a registrar and discuss payment options.

06 Swipe Card (Patients that choose to pay on the tablet):
- Swipe the credit or debit card when prompted.
- The Processing Payment message displays while the transaction is being completed.
- Review and sign the Healthcare Payment System (HPS) receipt the same as the eCapture forms are signed.

07 Rating Screen:
- App would allow the user to rate the check-in process experience (like a star rating, etc, we are open to your thoughts).
- This step will be optional.
- In addition rating, user will also have ability to type in comments about the check-in process.

Important:
- Keep things consistent. This means all graphic styles should work together.
- All of the graphics should have a similar feel and general aesthetic appearance

- Patients who visits hospital for treatment.

Judging Criteria:
- How well you plan the user experience and capture your ideas visually.
- How well you implement the challenge requirements.
- Not you would need to implement all required functionalities and items as written in the supporting document.
- Cleanliness of your graphics and design.
- Creativity and ease-of-use is key to the success as it must be engaging to users.

Final Fixes
As part of the final fixes phase you may be asked to modify your graphics (sizes or colors) or modify overall colors. We may ask you to update your design or graphics based on checkpoint feedback.

Submissions are viewable to the client as they are entered into the challenge. Winners are selected by the client and are chosen solely at the client's discretion.
